2008 Conference Registration Form

Items to take note of:

Conference Fee: Utah RWA Members = $149. For all other attendees= $159. Fees must be received by Sept 6th, 2008. After Sept 6th please add $10 late fee.

Guest Meal Fees: $40=Fri Dinner. $35=Sat Buffet. $15=Fri Midnight Choc Retreat. Guest total for all meals=$90.

Cancellations made prior to 09/06/08 will be refunded, less a $25 handling fee. After 09/06/08, fees are non-refundable. A $20 service fee will be charged for returned checks. For questions please contact Lesli Lytle at lmuirl@yahoo.com.

For Booksigning - Participating published authors who are not members of URWA must be registered for conference and ISBN must be provided by August 9, 2008.
